The annual membership — $1,750

Payable monthly, beginning with your Health Design session. Where your insurance covers physician services, we bill it on your behalf. Your membership never exceeds $1,750 a year.

Your year includes

• Your Onboarding Portfolio and Dr. Herbert’s review

• A ninety-minute Health Design session

• Your personal health report, 15 to 18 pages

• Check-ins at months 3, 6 and 9

• Coordination with your physicians, with your permission and as advised

• Dr. Herbert’s counsel, throughout your year

• Your annual review, and the year ahead
